The Case Planner will work in partnership with the socio-therapist to assure a safe and permanent outcome for every child, with a focus on case planning. Ensuring that permanency is achieved in a timely manner. Promoting the best interests of each child with a moderate to severe emotional disorder.
Salary $48,000
To link and directly provide biological parents/discharge resources with clinical and case management services.
Case Planners will work closely and collaboratively with all support departments, and provide case work counseling to the child(ren), birth parents and foster parents to ensure permanency is achieved in a timely manner.
Monitors the overall adjustment of children in the foster home and alternative placements (hospitals, RTCs, RTFs etc.) and on trial discharge.
Manage caseload and provide regular home visits, supervise family visits and documentation in accordance with ACS/agency standards in CONNX and Evolve Electronic Record Systems.
Conduct activities and provide services that promotes each child(ren)/youth’s safety, optimal health, development and wellbeing.
Monitor child’s health, school performance and adjustment to foster home.
Provide stabilization to the foster children.
Prepare for and attend monthly treatment planning meetings , family team conferences
Schedule office visits, home visits and family and sibling visits.
Provide casework counseling to foster parents, adoptive parents and birth parents.
Prepare case documentation in a timely manner (i.e. progress notes, psycho socials, FASP’s, Permanency Hearing Reports.
Case Planner will participate in an evidence informed model and maintain fidelity to the model.
Attend all court/legal proceedings, TPRs, trials and extension of Placement.
Attend clinical meetings, case management meetings.
Interview and counsel pre-adoptive parents on adoption procedures and policies.
Valid New York State driver’s license is strongly preferred.
Transport clients when necessary.
The Case Planner will be available for emergencies on weeknights and weekends and carry an agency cell phone.
Provide coverage where necessary for cases assigned to colleagues on vacation.

Bilingual (English/Spanish)Entry level position but Child Welfare experience a plus. Excellent communication and writing skills needed. Computer literate. Bilingual Spanish preferred.
