Bilingual Case Worker

About the Role: We are seeking a dedicated and compassionate Bilingual Case Worker to join our team. In this role, you will provide essential support to clients by helping them navigate challenges and achieve their personal goals. Your primary focus will be improving clients' quality of life through individualized support plans, counseling, and referrals to community resources. You will collaborate with a network of professionals to ensure clients receive holistic, high-quality care. Key Responsibilities: + Conduct client assessments and develop personalized care plans. + Provide ongoing counseling and emotional support to clients and their families. + Coordinate and monitor service delivery, ensuring timely and efficient access to resources. + Maintain thorough and accurate client records. + Collaborate with social service providers, healthcare professionals, and other community resources to ensure comprehensive client support. Qualifications: Minimum: + Bachelor’s degree in Social Work or a related field. + Fluency in both English and Spanish. + Experience working with diverse populations. + Strong communication and interpersonal skills. + Ability to work independently and collaboratively in a team environment. Preferred: + Master’s degree in Social Work or related field. + Experience working with older adults. + Familiarity with community resources and support services. + Proficiency in case management software. Skills You’ll Need: + Cultural Competency: Your experience with diverse populations will enable you to deliver culturally sensitive and respectful care. + Communication & Compassion: Use your bilingual fluency in English and Spanish to effectively connect with clients, offering empathetic guidance. + Problem-Solving & Coordination: Your ability to develop care plans and navigate resources will help clients achieve better outcomes. + Collaboration: Work in synergy with professionals across fields to ensure clients get the most comprehensive support.
