As a Bilingual Lead Care Manager, you will act as the primary contact person for youth and families requiring care coordination and community linkages. Bilingual Lead Care Managers play a pivotal role in providing comprehensive care management and support serving as the cornerstone of the care coordination process and the driving force behind ensuring the holistic well-being of high-need youth and families.

The Enhanced Care Management (ECM) program is a Medi-Cal managed care benefit that addresses clinical and non-clinical needs of high-need youth and families through the coordination of services, connection to community resources, and comprehensive care management.

Responsibilities Engage in outreach activities to enroll new clients in ECM, while building rapport with enrolled members.Complete a comprehensive assessment and develop a care plan for each enrolled member.Organize care activities and maintain regular contact with outside providers to ensure coordination, including county substance use disorder and specialty mental health providers as appropriate.Manage referrals, coordination, and follow-up to needed services and supports.Provide education and identify support needs for youth and their family or other caregivers.Provide services in person and/or via telehealth to meet each enrolled members’ care plan needs.Utilize crisis communication and de-escalation techniques as per Seneca's crisis response training. This may include intervening with the clients' behavioral program through physical management of the clients, if applicable. Training in additional crisis intervention modalities may be required.Engage in community outreach and identify supports and resources in the community, including building relationships with community providers.Complete required documentation in a timely manner. QualificationsREQUIRED:
Education/experience requirements:Bachelor’s degree in Social Work/Psychology/Counseling + 1 years of experience ORAssociate’s degree or equivalent + 2 year of applicable experience ORHigh School Diploma/GED + 3 years of applicable experienceExperience required working with families who are experiencing the impact of complex systematic disparities that contribute to community and individual mental health challengesBilingual Spanish skill set requiredFlexibility to work some earlier mornings or evenings, scheduled ahead of timeMust be at least 21 years of ageOwn vehicle, Valid California driver's license and insurability with Seneca's automobile policyTB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirementsMust show proof of COVID-19 vaccinationPREFERRED:
