The Social Worker will serve as part of an interdisciplinary team caring for patients at CommUnityCare. This is a grant-funded position ending on May 31, 2027, with the possibility of renewal based on funding availability, work performance, and progress toward established goals.
This role offers a hybrid work schedule, with certain duties performed off-campus at a personal residence located within 75 miles of a university-owned or leased property. Remote work requires reliable internet access and a suitable, distraction-free workspace. A flexible work arrangement agreement will be required upon hire.
This position represents a collaborative effort between the Division of Health Social Work and the Department of Internal Medicine. The Division of Health Social Work serves as a centralized hub for social workers and other integrated health providers, fostering an environment that empowers professionals to improve the health and enhance the well-being of patients and families in partnership with patients, families, and communities. Through cutting-edge models and comprehensive services, the division creates tangible value within the healthcare ecosystem while championing whole-person care and interprofessional collaboration. Functioning as a formalized collaborative structure, the Division of Health Social Work unites interprofessional missions, cultivating synergies that drive impactful, patient-centric care.
Purpose
The Social Worker will provide cancer risk-reducing behavioral assessments and interventions for various vulnerable patient population while fostering respectful, collaborative interactions with patients and their families. Additionally, the role involves building strong, patient-centered relationships and demonstrating a commitment to and competence in working with historically underrepresented populations.
Responsibilities
Apply empowerment-based prevention and intervention strategies including motivational interviewing to improve community health and facilitate screening, follow-up, and treatment for alcohol use.
Evaluate patients for additional cancer-related risk factors and provide referrals to meet those needs. Ensure timely receipt of screening, results, and scheduling for follow-up, as needed.
Facilitate patients requiring additional testing or treatment through patient navigation, including addressing barriers patients may experience. Address personal and environmental issues with the patient to maximize emotional, social, and physical well-being and the appropriate use of healthcare resources.
Provide training for providers and staff on new screening criteria and techniques for unhealthy alcohol use to increase referrals for decreasing unhealthy behaviors and/or receiving treatment for alcohol use disorder.
Work closely with the Program Coordinator, Manager, and Program Director of the Department of Medicine at the University of Texas Dell Medical School to ensure coordinated efforts and effective program implementation. Partner with clinic providers and support staff to assess patient needs and deliver comprehensive healthcare services, both in-person at the clinic and through telemedicine platforms. Develop and maintain strong internal relationships and partnerships with colleagues, including clinical managers, support staff, providers, and business office personnel, to foster a collaborative and supportive work environment.
Ensure all tasks provided and associated with patient care, including documentation of patient contacts and progress, patient administrative processes, and related duties comply with all regulatory and accreditation standards. Attend seminars and maintain all licensure and/or certification requirements to support ongoing education and adherence to best practices.

Required QualificationsBilingual (English/Spanish). Master of Social Work (MSW) degree from a CSWE-accredited program. Licensed as a Licensed Master Social Worker (LMSW) in the state of Texas. Demonstrated strong communication and collaboration skills with a wide range of healthcare professionals. Exhibits a positive, compassionate, and professional attitude when engaging with patients and their families. Possesses excellent interpersonal, written, and verbal communication skills, along with strong organizational and time management abilities. Strong analytical and problem-solving capabilities, with the ability to thrive in a fast-paced, multi-provider practice environment.
Relevant education and experience may be substituted as appropriate.
Preferred QualificationsLMSW in state of Texas and at least two years of post-degree clinical experience in a healthcare setting (medical navigation or case management) with a preference for candidates who have worked with patients experiencing alcohol use disorders or Licensed Clinical Social Worker (LCSW) in the state of Texas. Experience in alcohol screening, assessment, and treatment, including SBIRT (Screening, Brief Intervention, and Referral to Treatment) and motivational interviewing. Clinical and telemedicine experience. Demonstrated experience working within an interdisciplinary care team.
Salary Range$63,000+ depending on qualifications
