The Wraparound Social Worker, is responsible for both the clinical treatment and case management of six to eight youth and their families who are referred through social service, and probation. The Wraparound Social Worker will act in close partnership and a liaison with Department of Social Services-Child Welfare Services, Juvenile Probation, Behavioral Health, and other service providers providing strengths-based and outcome-oriented alternatives to out of home placements for the youth. Our Social Worker is part of a multidisciplinary team to support the youth in meeting all treatment goals to ensure permanency and stabilization in current placement while increasing lifetime connections with others in the community and family. Treatment and support services are tailored to address behaviors and meet family needs, even as they change over time.

Seneca offers intensive "Wraparound" services to children and youth at risk of placement in a high-level residential treatments or juvenile hall, and to their families. The Wraparound programs are strengths-based, family-driven, flexible, and creative, with the goal of helping the family develop the skills and supports to prevent or reduce the possibility of residential placement of their child. A family-based team addresses the needs of the entire family, not just the at-risk child. This family-based team consists of family members, Seneca's Care Coordinators, Youth counselors, Parent Partners, Permanency Specialist and other community members who have an important role in the family,

Responsibilities Function as the team leader and coordinate all treatment and WRAP activitiesMeet with clients and caregivers on a weekly basis to further treatment goalsDevelop and facilitate Child and Family Teams for the purpose of treatment planning and supportCultivate and maintain relationships with community members and providersAttend all program staff meetings, treatment reviews, IEPs, and other agency meetings as necessaryProvide direction to treatment team on clinical issues, casework issues, and written reports for each caseCoordinate and implement overall treatment and services for assigned clientsChart weekly progress notes and all other documents needed for treatment services, including assessments, diagnoses, and treatment plansIntervene with clients' behavior management programs as neededChart daily mental health notes and all other documents as needed, such as assessments, diagnoses, treatment plans, outcome measures, incident reports, and other documentation accurately and in a timely manner. Complete documentation as required, per Medi-Cal regulation and expectationsPart of the on-call rotation (stipend and over-time pay provided)Participate in individual and group supervisions QualificationsREQUIRED
Master’s degree in psychology/social work/counselingBilingual in Spanish/EnglishMust be eligible to register with California BBS.Valid CA Driver’s license, and insurable under Seneca Family of Agencies insurance policy.
