Incumbent is guided by a thorough knowledge of the psychosocial evaluation process, social work intervention, rules & regulations of the Dept. of Health, Education, Welfare, and the rules & regulations of the hospital **Responsibilities:** + Completes psychosocial evaluations + Completes collaborative treatment planning and implements patient centered services, involving family and /or care partners in the treatment plan process + Conducts individual, group psychotherapy as prescribed by the treatment plan + Participates in departmental meetings + Documents according to clinical standards + Develops after care plans for patients upon discharge including linkages to other agencies as appropriate + Proficiency in implementing Suicide Risk Assessment + Proficiency in implementing Telehealth Services Policy in compliance with Federal and + NYS regulatory guidelines **Required Skills:** + Master of Social Work + Valid NYS LCSW + Bilingual Spanish required + 2 + years of experience in Behavioral Health Outpatient setting Benefit offerings include medical, dental, vision, life insurance, short-term disability, additional voluntary benefits, EAP program, commuter benefits and 401K plan.
