Are you looking to make a difference in the lives of children and their families? We are hiring a Bilingual Case Manager for our Wraparound Program to ensure children and families receive comprehensive mental health services promptly.

 

 

As a Bilingual Case Manager, you will provide individual and family-based Wraparound care coordination, case management, mental health interventions, support, and crisis intervention, services to children, youth, and families who are primarily in the foster care and juvenile justice system. Your work as a Bilingual Case Manager will be essential to the treatment team staff members' collaborative efforts to provide services in the community, including, but not limited to, hospitals, schools, juvenile halls, Seneca offices, and family homes.

 

ABOUT THE PROGRAM

The Wraparound Programs work in close partnership with Department of Social Services, Juvenile Probation, Behavioral Health, and group homes. Wraparound provides strengths-based and outcome-oriented alternatives to group care placements for children and youth by drawing upon community and family strengths. The Wraparound team builds consensus and leverages the strengths of its members to identify and meet needs that require intervention to support the child/youth and family in reaching their long-term goals.

Responsibilities Meet with clients and caregivers on at least a weekly basis to further treatment goalsEngage in regular collaboration with family, natural supports, collateral supports, community resources, County Workers, and the Wraparound teamProvide case management/linkage for the client and their family and mental health intervention services to ensure the successful transition of clients and their families from Wraparound servicesParticipate in all client related meetings (such as IEPs, treatment reviews, etc)Complete curfew calls and curfew log to provide to treatment team for youth on probation as neededAssist with planning for transportation arrangements for school, therapy appointments, and court hearingsParticipate in crisis response on-call system as outlined by the programComplete progress notes and other client and program paperwork on time Qualifications

Required:

Education/Experience Requirements:Bachelor’s degree ORAssociate’s degree or equivalent + 1 year of applicable experience ORHigh School Diploma/GED + 2 years of applicable experienceApplicable experience can include paid or unpaid experience working with youth or families or working in the social services field. Bilingual in Spanish/EnglishBe part of a rotating emergency pager system, scheduled with supervisor ahead of timeMust be at least 21 years of ageOwn vehicle, Valid California driver's license and insurability with Seneca's automobile policyTB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirementsMust show proof of COVID-19 vaccination
