As the Bilingual Facilitator you will be a vital part of our team, you’ll lead Child and Family Planning Meetings, collaborate with community and county partners, and provide crisis intervention when needed. Your ability to build strong partnerships and navigate complex situations will be crucial to ensuring comprehensive care for clients and their families. You will gain valuable experience working in a collaborative, strengths-based team while growing your expertise in community-focused care and providing in-person services within the San Jose community.
ABOUT WRAPAROUND
The Santa Clara Wraparound program supports Santa Clara County children and adolescents in the Child Welfare, Probation, and Mental Health systems who are transitioning from or at risk of entering out-of-home care or juvenile hall. Aimed at keeping youth in family-like settings, the program serves youth aged 5-18 and occasionally youth 18-21 years of age. Team members work with children and families providing services almost exclusively in the community: in families’ homes, schools, and other community locations within a 90 mile radius of our program in San Jose, CA.

Responsibilities Facilitate Child and Family Planning Meetings, including preparation, coordination, facilitation, note-taking, and follow-upManage cases and coordinate care with county and community partners (e.g., social workers, therapists, probation officers, teachers)Complete all necessary documentation, including mental health notes, Family Team Meeting notes, and incident reports in a timely and accurate mannerProvide crisis management and intervention, including behavior management if necessaryAttend team meetings, trainings, and supervision sessionsFollow clinical best practices and complete required clinical documentationParticipate in emergency on-call response once every quarterUtilize crisis communication and de-escalation techniques as needed QualificationsREQUIRED
Bachelor’s degree2 years’ experience working with children/youth and families with complex and enduring needs in a mental health settingBilingual Spanish skill setFlexible schedule with the ability to work some evenings and weekend hours as neededBe part of a rotating emergency on-call system, scheduled ahead of timeMust be at least 21 years of ageValid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policyTB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirementsProof of up-to-date COVID-19 vaccinationPREFERRED
