Focuses on decreasing time from diagnosis to treatment and preventing treatment delays. Educates/provides information and supports patients and their families while facilitating a treatment plan. Functions as a patient advocate, guides patients and families throughout cancer care and survivorship experience. + Tracks patients via the navigation system and the NCCN algorithms. Identifies where the patient is in the cancer continuum and anticipates the next step in the plan of care. + Assesses cancer patients to anticipate and identify necessary support along the cancer continuum. Provides access to community resources and links patients to support services such as: social services, nutrition services, psychologist, pastoral care, physical therapy, rehabilitation therapy, financial counseling, national and local programs, and resources, etc. as expected in NAPBC standards. + Develops and maintains relationships with healthcare providers, oncology providers and other outpatient and inpatient support services and departments. Acts as a liaison and facilitates communication regarding the plan of care and navigation services. Assures referral to the navigation team is complete and aides in the prior authorization process. + Actively participates in and helps to organize the Tumor Board and the multidisciplinary meetings. + Coordinates high risk identification program, post treatment ancillary and support service, and survivorship programs. + Develops and implements in-house and community programs/workshops/seminars/support services. Participates on the delivery of appropriate outreach programs. Works directly with AJH marketing personnel to evaluate and promote the Breast Care Center. Maintains data on referring physicians to Breast Care Center. + Effectively engages the patient and families to provide support while assessing their emotional, physical, psychological and spiritual needs. + Maintains familiarity with the NAPBC standards and ensures data base for NAPBC is complete and up to date. Maintains performance improvement. + Monitors and reports on patient satisfaction data for breast care program. Helps to identify and implement solutions for problems identified in patient satisfaction. **Scheduled Hours:** 40 hr/wk **Work Shift:** M - F 8a - 430p (some flexibility with schedule). **Education Required:** High School Diploma or equivalent required. Graduate of an accredited nursing program preferred. **Experience Required:** 3-5 years oncology nursing or navigational experience; OCN preferred. **Licensure/Certification Requirements:** Current Massachusetts license as a Registered Nurse preferred. Willingness to be certified as a Certified Breast Care Nurse (CBCN) after 1-year experience.
