Family SupportProgramis looking for visionaries eager to utilize an evidence-informed model to support families to navigate, survive, and thrive in their worlds. Join us in this ground-breaking initiative to provide an evidence-informed intervention called Solution Based Casework (SBC). In addition, we provide to families case management services, advocacy, referrals, assistance with childcare, housing, income support, food pantries, and medical and mental health services. The Solution Based Casework (SBC) Modelprioritizes working in partnership with families, focuses on pragmatic solutions to difficult situations, and celebrates family successes. Through the SBC Model Case planners are to help strengthen families, stabilize, and maintain family functioning, while continually assessing for risk and safety issues. Services are provided to at-risk families referred by the Administration for Children’s Services or self-referred. Under the supervision of the Supervisor, the Case Planner will be responsible for case intake, management of cases and providing service needs, in accordance with ACS standards and indicators and Rising Ground policy and procedure manual. Responsibilities · Responsible timely submission of all paperwork, completion of intakes as well as progress notes and all necessary paperwork · Coordinate all case management, referral and advocacy services for each family. · Develop resources for social services referrals, assessing, coordinating and evaluating preventive services needed by the family · Provide individual, family and group counseling. · Maintain case records in an orderly fashion. · Home visits must take place weekly and may require case planner to walk up steps. Evening hours may be warranted for some cases. Qualifications + Bachelors degree or Masters Degree in Social Work or in related field + Good writing and oral communication skills; computer literate + 2-3 years experience in child welfare field strongly preferred + Excellent time management skills with the ability to meet deadlines in a timely manner while managing competing priorities + Bilingual, English/Spanish to conduct case management for our families. + Excellent written and oral communication skills + Proficient use of Microsoft applications; knowledge of PROMIS/CONNX desired + Cultural awareness and sensitivity when serving clients that come from different backgrounds and walks of life Locations: Long Island City, Jamaica and Far Rockaway (Queens, NY) Salary Range $50,000.00 - $53,000.00 Incentives available if bilingual or licensed MSW or MHC EOE Job LocationsUS-NY-Queens Posted Date2 years ago(6/7/2023 12:45 PM) Job ID 2023-2497 # of Openings 5 Category Other
