Identifies reportable cases, analyzes and abstracts clinical data, codes, and enters the data into an electronic database after reviewed and approved by a Certified Tumor Registrar (CTR) until fully trained and defined accuracy is achieved. Performs lifetime follow-up to provide cancer incidence, treatment and outcome information.
Job Duties Identifies malignant, reportable-by-agreement benign or borderline disease cases as defined by the American College of Surgeons Commission on Cancer,PA Cancer Registry ) and Geisinger Cancer Committees for suspense file using multiple search methodologies.Reviews and interprets information in electronic and hard copy medical record to extract and maintain data in accordance with Geisinger Cancer Center operational standards as well as state and national reporting requirements after reviewed and approved by a CTR.Abstracts primary and recurrence and metastasis site data including demographics, diagnosis, history of cancer, co- morbidities, stage and extent of disease, diagnostic procedures, treatments and provider information utilizing the current Facility Oncology Registry Data Standards manual after reviewed and approved by a CTR.Inputs information directly into the computerized cancer registry.Codes stage of disease using the TNM staging system in the appropriate American Joint Commission on Cancer, Staging Manual after reviewed and approved by a CTR.Codes disease indicators morphology and topography for solid tumors using the International Classification of Diseases for Oncology, 3rd Edition (and Multiple Primary and Histology Manual after reviewed and approved by a CTR.Codes extent of disease and Site Specific Factors using the Collaborative Stage Data Collection System Coding Instructions Manual after reviewed and approved by a CTR.Maintains accession file on all patients entered into the cancer registry.Performs follow-up for all living patients on an annual basis to determine current disease status in accordance with FORDS guidelines.Completes and closes cases on expired patients.Maintains non-reportable and suspense file using Suspense Access application.Reports data to PCR via appropriate media in accordance with reporting deadlines.Assists CTR and Manager, Cancer Registry with preparation for Cancer Center surveys as needed.Assists with follow-up and case finding training of new cancer registry personnel.Supports quality improvement activities to maintain and improve cancer patient care.Utilizes the 05 Hematopoietic and Lymphoid Neoplasm Case Reportability and Coding Manual and the Hematopoietic Database to determine reportability, code cases with reviewed and approved by a CTR, and determine multiple primaries with review and approved by a CTR.
