Position Summary

 

Cardiovascular Outpatient RN for Cardiac and Pulmonary Rehab: The Registered Nurse, under the direction of the Manager, assesses, develops and implements plan of care given as well as the response measured from the exercise prescription.  Additional responsibilities include patient education concerning cardiac and pulmonary risk factor reduction education, activity progression and dietary modification for adults 18 and older.

 

             

Minimum Requirements

 

Education        

Graduate of an accredited nursing program Associates Degree in Nursing

 

Experience      

Minimum of 2 years cardiac experience

 

License/Registration/Certifications 

Current license to practice Nursing in the State of South Carolina Certification in Basic Life Support Cardiac Rehab and Outpatient Heart and Vascular Center only: Advanced Cardiac Life Support

 

Preferred Requirements

 

Preferred Education  

Bachelor’s Degree in Nursing

 

Preferred Experience

Cardiac Rehab: 1 year of Cardiac Rehab experience

 

Preferred License/Registration/Certifications          

Cardiac Rehab: Certified Cardiac Rehab Professional

Assures continuity of care through effective decision making resulting in quality patient care Documents all aspects of patient care by following established charting procedure to show the patient’s progress or any change in the patient’s condition Consistently follows all regulatory guidelines for the department and hospital. Completes all documentation requirements thoroughly and in a timely manner per hospital requirements Communicates effectively with all members of the care delivery team. Responsible for participating in process improvement projects within the department. Provides excellent customer service and effective communication with both internal and external customers of the department. Serves as a clinical resource within the department. Ensures that appropriate critical path/standard of care/plan of care is identified and collaboratively implemented. Adhere to and practice appropriate infection control policies and procedures. Perform accurate, legal, and ethical documentation at all times. Other Duties as assigned

 

Core Job Responsibilities-Cardiac Rehab Nurse

 

Successful completion of an Arrhythmia course. Completion of critical care course desired. Orientation for new patients including creating initial exercise prescription and developing Individual Treatment Plans Serves as a clinical resource within the department Ensures that appropriate critical path/standard of care/plan of care is identified and collaboratively implemented.  Consistently follows all regulatory guidelines for the department and hospital.   Completes all documentation requirements thoroughly and in a timely manner per hospital requirements.   Communicates effectively with all members of the care delivery team.  Assists with inpatient consultations and education and assisting with referrals as assigned.
