Position Title: Cardiac Rehabilitation Coordinator

Position Summary:

Organizes, implements, manages and evaluates the Cardio/Pulmonary Rehab program. Implements and supports each component of the process to ensure the client's plan of care in partnership with the primary care physician and other providers of Trinity Health.

What you will need:

Licensed to practice as a Registered Nurse in the State of Michigan and/or degree in exercise physiologyACLS preferred and/or education in exercise specialtyExperience in Cardio-Pulmonary Rehab preferredExperience and knowledge of cardiovascular disease, emergency procedures, nutrition, physiology, health education, psychology, medical/educational strategies for CAD risk factor management


What you will do:

Implements the process of assessment, planning, treatment interventions, ongoing evaluation of the client and the plan of care.Collaborates with the team, manager and physician to ensure quality care.Communicates effectively with the other member of allied health team and public.Monitors, evaluates and reports B/P, THR, oxygen saturation, SOB and RPE scale, as well as other S & S during exercise.Communicates assessment concerns and effort symptoms to the physician and team when indicated.Follows department policy for emergencies and treatment of effort symptoms.Maintains accurate and current records of each patient's plan of care and critical path progress.Completes documentation including patients' records, notes, assessment, goal establishment and outcome monitoring, education, variance reporting and orientation record and charge process.
