Care Base Advanced Practice Provider

With our combined resources CommonSpirit is committed to building healthy communities advocating for those who are poor and vulnerable and innovating how and where healing can happen both inside our hospitals and out in the community. **Responsibilities** This is a remote position. **Job Summary** This position will be deployed within the Care Base Infrastructure to manage patients and provide clinical oversight during transitions of care, clinical escalations from home health RN teams, Care Base nursing teams or any potential escalation points during virtual services across the continuum. This virtual Advanced Practice Provider role will be leveraged to provide additional clinical expertise to ensure excellent quality care delivery for a group of specified patients within the Care Base via asynchronous care, advice, or virtual visit and transition back to primary care, specialty care team members or escalate care within CommonSpirit Health as appropriate. This position would serve as a critical access point for patients to be evaluated by a qualified provider, improving overall patient outcomes and satisfaction, reducing readmission rates and Skilled Nursing Facilities, interacting with Home Health nurses and providers, Emergency Department (ED) visits, improving access to outpatient clinics and assisting move patients through acute spaces efficiently. **Essential Functions** + Serves as a Care Base leader and liaison with healthcare partners + Develops and maintains a cooperative and collaborative working relationship with the integrated care delivery teams and partners to develop new patient pathways to access healthcare when escalation scenarios occur + Develop and implement treatment plans that are aligned with the practice preferences of the collaborating physician(s) including orders for medication and/or other treatment modalities, the need for additional testing, consultation with other providers, and follow-up provisions. + Review clinical information and identify interventions (ex., evaluation of pending labs or other diagnostic tests and/or medication adjustments that may be medically necessary for referred patients). + Perform brief and targeted history and physical assessments as applicable via telehealth and virtual/telephonic modalities of patients that are accurate, concise, relevant, and appropriate + Orders, reviews and interprets diagnostic, radiologic, and laboratory tests as appropriate to the patient’s condition and specialty + Documents information in the electronic medical record according to policy and timeliness standards. This includes history and physical exams (H&Ps), focused progress notes, and test interpretation. + Facilitates consults with and/or patient referral to the appropriate physician, specialty service, or agency + Completes applicable forms/documents based on practice specialty + Effectively communicates with patients and responds to messages within established time frames + Delivers care that is patient-centered, compassionate, appropriate, and effective, with emphasis on an efficient and comprehensive approach + Maintains applicable state licensure, national certification(s), DEA registration and applicable specialty certifications (i.e.BLS, PALS, ACLS, DOT, Federal Drug & Alcohol testing). Obtain and maintain discipline-specific pharmacology and controlled substance required training and/or certifications per state regulations + Adheres to organizational policy regarding prescription and surveillance of opiates and controlled medications by utilizing CommonSpirit Opioid Toolkit + Attends required percentage of quarterly, departmental, and mandatory/compliance training meetings + Oversight of telemonitoring/ RPM services in a variety of settings + Participates in quality assurance review of patient Inbound/Outbound calls and patient satisfaction surveys. + Provides care with attention to safety, efficiency, and cost-effectiveness. + Participates in training and staff development + Constantly demonstrates and maintains a high standard of confidentiality + Assists in community resource development + Assists in the development of new protocols + Understands challenges and demands of Healthcare reform **Qualifications** **Required Education and Experience** + Masters Degree as a Physician Assistant (PA)-C or a Master’s degree as an advanced practice registered nurse. + Minimum of 2 years experience in Advanced Practice clinical role in the acute or family/ internal medicine space. + Geriatric or Complex care experience is required. **Required Licensure and Certification** + Licensure (multi-state licensure preferred) and Board Certification an Advanced Practice Role: Physician Assistant (PA) or Nurse Practitioner (NP) **Required Minimum Knowledge, Skills, Abilities and Training** + Demonstrates excellent interpersonal skills with respectful, advanced level of communication skills (oral and written) and interdisciplinary collaboration + Provides excellent Customer Service + Must have a working knowledge of hospital throughput, continuum of care, and patient management post-discharge. + Must have excellent training in ambulatory care operations, including primary care, specialty care, and transitional care environments + Must have an aptitude and willingness for leadership of teams + Must show skills in the use of virtual technologies + Must have strong relationship-building skills with an ability to develop and maintain rapports with key stakeholders across the health system enterprise + Must be able to break down complex tasks into manageable parts in a systematic, and detailed-oriented manner. + Must show ability for problem-solving using solutions-oriented thinking. + Must be able to effectively handle (?) complex patient management across multidisciplinary groups. + Must be skilled in change leadership and in conflict management. **Pay Range** $49.50 - $65.00/hr We are an equal opportunity/affirmative action employer.
