Job Highlights:

This position is onsite/in person Monday through Friday in Madison during the first few months of training. After training, the schedule will transition to hybrid – 2 days onsite and 3 days from home. Candidates must be local to Madison or the surrounding area.

Job Summary:

Responsible for detailed preview of charts for high complexity patients and patients in the population health programs. Works with providers across the organization to ensure accurate coding for diagnosis. Assists in providing cost-effective healthcare services through the treatment process. Serves as a liaison between patients, physicians and all other participants of the health care team.

Job Responsibilities and Requirements:

PRIMARY RESPONSIBILITIES

Reviews charts to identify potential diagnosis codes so that risk factors can be properly addressed by physician during a risk assessment visit.Conducts audits including research diagnosis and health maintenance treatment.Schedules appropriate transition of care and assists licensed personnel to enhance comprehensive patient care and reduce gaps.Identifies ongoing health maintenance needs and facilitates next steps for those needs.Performs other duties as assigned.

High School diploma/GED or 10 years of work experience

PHYSICAL REQUIREMENTS

Constant standing and walking.Frequent use of hearing and speech to share information through oral communication.Frequent use of hearing to distinguish body sounds and/or hear alarms, malfunctioning machinery, etc.Frequent lifting/carrying and pushing/pulling objects weighing 0-25 lbs.Frequent reaching, gripping, and keyboard use/data entry.Frequent use of vision for distances near (20 inches or less) and far (20 feet or more) and to identify and distinguish colors.Occasional bending, stooping, crawling, kneeling, sitting, squatting and repetitive foot/leg and hand/arm movements.Occasional use of vision to judge distances and spatial relationships.Occasional lifting/carrying and pushing/pulling objects weighing 25-50 lbs.Occasional lifting/moving patients.Occasional use of smell to detect/recognize odors.Occasional driving.
