Position Title : Care Coordination Supervisor (Bilingual: Spanish) Job Responsibilities: + Ensure all company policies and staffing protocols are met + Oversee daily dashboard to make sure attendance is confirmed and patients are safe in their homes and taken care of + Manage a team of Scheduling Coordinators; review and approve PTO, complete performance evaluations for the team, maintain coverage when a Coordinator is absent + Assist the Coordinators and Team Leads with difficult-to-staff cases; assist Coordinators with resolving any complaints, Incidents and or problems whenever needed + Responsible for the case assignment to the Coordinators on the team + Be the point of contact to all contracts + Monitor patient satisfaction and following with any patient when complaints are made + Document and manage any patient activity and report it + Work collaboratively with colleagues and participate in weekly Coordination meetings + Possess the capability to implement strategic thinking when assessing cases from contracts + Follow up with a contract when required; communicate any changes in scheduling such as replacements and any incidents that have occurred. + Monitor patient satisfaction and follow up with any patient when complaints are made + Monitor interdepartmental correspondence to ensure proper and complete follow up on behalf of Coordination department + Counsel PA's when the incident is beyond a Coordinator's scope of practice + Monitor PA electronic verification emails to ensure that attendance is being verified + Complete daily/weekly/monthly logs for tracking the department's performance + Manage the information between the patient and the aide and assist with communication between departments and overall office operations Job Requirements: + High school graduate; some college preferred. + 3+ years experience in other work related experience, preferably within the health care services or homecare industry. + 2+ years in a Supervisory or Management role + Proficient in Microsoft Office Suite, Constituent Database (HHA eXchange), Medflyt + Understands the regulations governing the home care field, related to Medicare, Medicaid and other insurance. + Understanding of communicating effectively with employees, patients and their families, medical and community affiliates in order to develop positive relationships + Understand the administration and management of office operations for home care agencies + Ability to perform various computer functions for information concerning patient and aide scheduling and coordination, assisting with communication between departments and overall office operations. + Proficient in Customer service and satisfaction + Bilingual English/Spanish True Care provides equal employment opportunities to all employees and applicants for employment without regard to race, religion, color, ethnic origin, gender, gender identity, age, marital status, veteran status, sexual orientation, disability, or any other basis prohibited by applicable federal, state, or local law. #hp Employment Type Full-Time Compensation $26/hr - $28/hr
