Broward House, Inc. is a not-for-profit community-based organization serving Broward County with a focus on HIV care and Prevention, housing, and behavioral health.  Broward believes in the dignity and value of every person who walks through the door. This position is responsible for assessing, coordinating medical and support services and preparing community clients for the highest attainable level of independence. 

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

 

Completes a bio-psychosocial assessment and ongoing assessment of client’s strengths, needs, abilities and preferences.Develops an individual plan of care and/or transition plan based on identified client’s needs.Engages, counsels, manages and effectively works with persons in crisis.Engages, counsels, manages, motivates and effectively works with high-risk, vulnerable client population including chemically dependent persons.Engages clients in a strength-based supportive manner providing re-directions and guidance as necessary.Promotes program refinement, understands client/public need and effectively participates in all assigned meetings.Submits all documentation in an accurate and timely manner.  Achieves and maintains program established productivity levels.Meets with internal and external interdisciplinary treatment team members to coordinate client services and documents outcomes of meetings, including hospitals, health department and medical providersArranges for services and completes linkage required for client based on individualized needsOther duties as assigned. 

KNOWLEDGE, SKILLS, AND ABILITIES:

Effective verbal and written communication skills Ability to effectively communicate with people at all levels and from various backgrounds.Solution focused approach to conflict and challengesAbility to maintain a strong sense of respect for confidentiality involving both clients and fellow employees.Ability to establish and maintain effective working relationships with employees, collaborating agencies and the publicAbility to demonstrate and model culturally competent behaviorModerate to Advanced Computer SkillsAbility to work independently with minimal directionBasic office equipment operation skillsKnowledge of Substance Abuse and Mental Health Symptoms and basic interventionsAbility to effectively engage with high-risk, vulnerable client population and persons in crisisAbility to legally operate and motor vehicle and provide own transportation.EDUCATION AND WORK EXPERIENCEBachelor’s degree in a social service or health related fieldOr Licensed Practical Nurse with 2 years case management/social services experienceValid Florida Drivers License and ongoing proof of insurance required
