The Care Coordinator provides intensive clinical care coordination to the youth and family as well as counseling and behavioral / emotional support.  Your primary role as the Care Coordinator is to coordinate care among all entities involved with the child, ensuring everyone is aligned with the goals, and help build natural supports and enhance the treatment team. Care Coordinators engage clients in meaningful and authentic relationships while providing a safe, predictable, and positive environment that aligns with our clients’ values and cultures. Services will be provided almost exclusively in the community in families’ homes, group homes, foster homes, schools, etc.

Seneca serves Santa Clara County children / youth by offering a focus on managing mental and behavioral health needs and increasing placement stabilization. The Katie A. Services program provides culturally-competent, team-driven, and strength-based unconditional care for children and families who struggle with profound needs and difficulties. Katie A. Services services dually focus on the child and family team meeting process that focuses on building a strong team to collaboratively address the needs of the child and family. Services are provided at the program site and in the community, including schools, and family homes.

Responsibilities Promptly meet with clients and caregivers on a regular basis to further treatment goals utilizing culturally informed and best practice modalitiesCultivate and maintain relationships with community members and providersComplete assessment and evaluation process of clients in order to develop and implement the care planProvide assistance in case planning and consultation to team members on clinical issues, as well as casework issues, as appropriateChart mental health notes, care plans, outcome measures, incident reports, and other documentation accurately and in a timely mannerMaintain availability for crisis intervention services, such as on-call rotation, if applicableParticipate in individual and/or group supervisionMaintain strict confidentiality of client and business operations information Qualifications

REQUIRED

Master’s degree in social work/psychology/counselingRegistered with California BBS: ASW, AMFT, APCC, or full licensedFlexible schedule with the ability to work some evenings and weekend hours as neededMust be at least 21 years of ageValid driver's license, clean driving record, and insurability through Seneca Family of Agencies’ insurance policyTB test clearance, fingerprinting clearance, and any other state/federal licensing or certification requirementsProof of up-to-date COVID-19 vaccination

PREFERRED

Bilingual in Spanish/English Experience working with children/youth and families with complex and enduring needs Experience implementing family-focused services
