The Care Coordinator advocates on behalf of the client, making linkages/referrals for services, coordinate services and collaborating with other agencies for continuity of care for children exposed to sexual assault, domestic violence, dating violence and/or stalking and their non-abusing parent.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES:

Provide direct services to those identified as children exposed to violence and/or their non-abusing parent Coordinate services provided by ancillary agencies: linkages and referrals. Conducts Needs Assessments Prepares service plan documents Development of the discharge plan in coordination with other treatment team members Provide sessions with respect to case management and Service Plan development Responsible for maintaining the clients chart in compliance with licensing, CARF, Medicaid, and other regulatory standards. Attend and participate in project related training, interdisciplinary team meetings, staff meeting and community task forces Perform other duties as assigned by the Director/Supervisor Understanding and application of federal, state and local confidentiality rules and professional standards of conduct
