As a Care Coordinator, you will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners.

In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plansLink patients with service providersCoordinate and monitor care plan deliveryEstablish a helping relationship with patients and their familiesBalance patient needs and choices with available resources, ensuring patients’ values and preferences are respected

 

What must you have?

Membership, in good standing, with the applicable regulatory body:College of Nurses of OntarioCollege of Physiotherapists of OntarioCollege of Occupational Therapists of OntarioOntario College of Social Workers and Social Service WorkersCollege of Dietitians of OntarioCollege of Audiologists and Speech Language Pathologists of Ontario2+ years of recent experience in community health or a related fieldKnowledge of the health care delivery system and community resourcesExcellent interpersonal, communication, assessment, problem-solving, and decision-making skillsEffective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environmentEstablished ability to accurately complete required documentation, reports and formsA valid driver’s licence and access to a reliable vehicleProficient in a Windows environmentWe have a mandatory COVID-19 vaccination policy. As a condition of employment, all employees are required to submit proof of COVID-19 vaccination status prior to start date.

What do I need to know?

Anticipated Start Date: January 13, 2025

Status: Temporary Full-Time (approx. seven months with the possibility of an extension)
Hours of work: 830am-430pm (Monday to Friday)
Site: Sarnia

Community/Complex Care Coordinators travel in the Sarnia-Lambton area to meet and collaborate with patients and their families to develop individual plans for service provision. Community Care Coordinators work a hybrid work model, subject to change.
