As a Care Coordinator for our WISe Program in South Seattle, Tukwila and South King County, you’ll have the opportunity to make a profound impact on the lives of children and families by guiding them toward stability and well-being. You will work closely with families to develop and implement personalized care plans, always ensuring that their voices are heard and their needs are prioritized. By collaborating with a multidisciplinary team of professionals, you’ll address the various aspects of a child’s and family’s life, from their home environment to schools and community settings, making sure they receive the support they need to thrive. Your services will be delivered where families feel most comfortable—whether in their homes, foster homes, or schools—offering direct, in-home support that fosters stability and growth. Acting as a central point of communication, you’ll be a lifeline for families, helping them navigate difficult transitions with consistency, clear communication, and a constant focus on their well-being. Your dedication will ensure that each family has the resources, guidance, and care they need to overcome challenges and move toward a brighter, more stable future.

The WISe program provides comprehensive behavior health supports to youth with complex behavioral and mental health needs. The program's goal is to help youth thrive in their homes and communities and assist in helping them meet the goals they have identified.

Responsibilities Build, plan, facilitate and communicate Child and Family Team (CFT) meeting details with WISe team members, clients and familiesMaintain a caseload of 10 -15 clientsEnsure to incorporate the input and needs of the youth when forming the teamEstablish and sustain an influential team culture by inviting CFT members to propose, discuss, and accept ground rules for working togetherOver time, work to transition facilitation to allow for youth and/or family members to be facilitators of their meetingsCultivate and maintain relationships with community members and providersUtilize consensus-building techniques to meet the needs of the youth and familyProvide assistance in case planning and consultation to team members on clinical issues, as well as casework issues, as appropriateMaintain team focus on the scope of work for the progress/movement toward transition, acknowledging and celebrate successes and changesStay abreast of all up-to-date Cross System Care Plans for each client and any other related documentationWilling to respond to crises and be part of a rotating on-call system Qualifications

REQUIRED

Bachelor’s degree (preferably in social work, psychology, or counseling)
Must be 21 years of age or olderIn person work and required travel throughout South King County, South Seattle, and Tukwila Flexibility to work evenings one to two times per week, and weekendsClearance of TB test, fingerprints, and any other state or federal requirementsOwn vehicle; valid Washington Driver’s License and insurability by Seneca's automobile insurance carrierProof of COVID vaccination

PREFERRED

Experience in WISe or other WRAP programsBilingual skills

Ability to work as part of and supervise a multidisciplinary teamStrong coordination, organizational and writing skills Schedule Full-time10:00 am- 6:00 pmMon-Fri.
