Job Description

The Care Coordinator is responsible for providing comprehensive support services to clients and their families. This role involves coordinating care plans, facilitating access to services, and ensuring clients receive the necessary support to achieve their health and wellness goals.

Pay and Benefits

The pay range for this position is $20.50 - $20.50/hr.
