Amergis Healthcare Staffing is hiring Care Coordinators in Chicago, IL!
$30 OR $35 / hour
Position: Care Coordinator – Aging Population
Location: Chicago, IL
Pay Rate (Local): $30 for unlicensed, $35/hr for license social workers
Schedule Options:
8am-4:30pm Monday to FridayWeekend Requirements: None
Duration: 3 months with extension based on performance
Minimum Requirements:
Bachelors degree in Social Work, Social Services, or related field from accredited college. Minimum 2 year of recent experience providing services and support to elderly or disabled population in a community based setting Must have experience traveling to members homes and facilities and doing risk assessments and treatment plans.
