Care Coordinator Assistant

Clinical Care Coordinator 1

Job Description: Primary responsibility is to make 25+ outreach calls per day to Medi-Cal members to offer care management services.  Each member

requires at least three outreach calls and one letter to offer care management services within a week. The person will document each call attempt, send a

letter, and document call outcome into the Care Connect system with notes. If the member is unable to reach after three call attempts, then the case will be documented and closed in the care connect system using our processes.  If the member is reached, the person will complete a short questionnaire with the member on the phone. then the member will be offered care management services using a script and sent to a nurse for follow up.  The person will be required to organize their case load of members to ensure that each member has been outreached to at least three times prior to closing.  We are using excel spreadsheet to track calls and outcomes as well as the Care connect system. The person will be asked to check other internal systems for contact (phone numbers) for members on our service. 

Software Skills Required: Microsoft Teams, Excel, Care Connect EMR system

Education Required: High School Diploma or Equivalent

Required Experience:

One year working in a managed care environment or health insurance Excellent telephonic communication and customer service Ability to use and document in multiple computer systems efficiently

Preferred Skills (Nice to Have):

Experience working with Medi-Cal population Working knowledge of community resources

Work Environment: Fully Remote - MUST LIVE IN CALIFORNIA

Schedule: Monday - Friday, 8am to 430pm

Contract Term: 6 months to start - high probability of being extended or brought on direct

Pay and Benefits

The pay range for this position is $18.00 - $20.00/hr.
