Position Responsibilities:

·       Schedule, Re-schedule or cancel all outside appointments including but not limited specialists, imaging, labs, surgeries etc. as directed by physician orders and or Clinic Manager for all participants.

·       Confirms outside appointment with the provider and verifies all needed information (auth, order, H&P etc.) has been received.

·       Accurately enters all required  information into Authorization, Order and Schedule screen per Eldercare protocol.

·       Works with Medical Record Coordinator to ensure documentation received on all outside appointments, ER and hospital stays and check for any follow up orders or recommended procedures and communicates this information to Medical Director, Provider and Clinic Manager verbally and via EMR Task.

·       Provides and explains all required preps/orders for the procedure that is authorized by Eldercare to the outside provider.

·       Communicates all pertinent information regarding the cancellation of appointments to the IDT Team and Clinic and documents such in EMR.

·       In scheduling comments enters all special requests, transportation and any other specific information related to the order.

·       Communicates to Clinic and IDT all outside appointments daily.

·       Schedule block appointments for Clinic and other IDT members as requested.         

·       Communicates with Clinic and IDT if any required tests or imaging requests have not been received that impact the schedule on a daily basis.

·       Acts as a back up for all other scheduling and  authorization needs inclduing but not limited to home care, DME, supplies, SNF, ALF etc

·        Reviews and process all outside specialists orders and other orders as directed within 5 business days.

·       Monitors all visit status to ensure the work flow is current.  Removes or moves all orders to the correct status.  Any orders in the workflow that are out of compliance with Eldercare protocol will be reported to  the supervisor.

·       Ensures that telephone lines are answered within established time frames.

·       Maintains compliance of participant Access Processes and Federal, State and Local Laws and regulatory standards (ACHA, HIPAA, Medicare, Medicaid, EMTALA, COBRA, etc.)

·       Assists Clinic as needed

·       Coordinates all transportation needed for scheudled appointments. This includes ensuring an Escort is assgined when needed. Reviews with Transportation Manager  as needed

·       Prepares daily updates for Clinic Huddle and attends the Huddle        

·       Assists with the review of the Pend report and corrects/updates authorizations as needed

·       Reconciles authorizations completed monthly to ensure all authorizations are removed for services that were canceled or otherwise did not occur.

·       Communicates and reminds (as needed) the participant off all outside appointments.  Identifies if transportation or an escort is needed.

·       Participate in and support Quality Improvement initiatives 

·       Know and adhere to philosophy and goals of Eldercare    

 

Qualifications:

·       Florida LPN License preferred.

·       High school graduate or equivalent level of training. Some college or Medical Assistant preferred

·       At least two years practical experience in computer usage. At least two years prior experience in scheduling, patient registration, authorizations and insurance verifications preferred. Managed care, hospital, physician practice or other health care business experience is very helpful.
