The Care Coordinator provides collaborative planning, referral and monitoring of treatment and support services to adults with severe mental illness, people with chronic addictions, or children with severe emotional disturbances and their families.   Your exceptional skills
2 years of experience in a position requiring training and case management preferred.   Your exceptional qualifications
• Bachelor's degree in Psychology, Social Work, or related field. (Required)
• Driver’s license and proof of auto insurance; driving record that adheres to CHI policy. (Required) • Must have recovery coach certification or ability to obtain one within 90 days   Your life with Community
