BCforward is currently seeking a highly motivated Care Coordinator II for an opportunity in Remote FL!


Position Title: Care Coordinator II


Location: Remote FL


Anticipated Start Date: 10/21/2024

Expected Duration: 6+ Months of contract with possible extension

Pay Range: $24 - $25/hr

Please note that actual compensation may vary within this range due to factors such as location, experience, and job responsibilities, and does not encompass additional non-standard compensation (e.g., benefits, paid time off, per diem, etc.).


**Position Purpose:**
The Care Management Support Specialist plays a crucial role in supporting care management activities and the teams assigned to members. This position ensures that services are delivered effectively by healthcare providers and partners, achieving continuity of care and high member satisfaction. The role involves member outreach through telephone or home visits, documenting care/service plans, and addressing any questions or concerns related to recommended care.

**Key Responsibilities:**

- Conduct outreach to members via phone or home visits to engage them in their care/service plans, discussing next steps, available resources, and ongoing education.
- Coordinate care activities in alignment with care/service plans, collaborating with healthcare and community providers, as well as members and caregivers, to accommodate changes or progress.
- Address various member and provider inquiries related to care/service plans, providing support as needed.
- Communicate with care managers, practitioners, and other stakeholders to facilitate member services and ensure continuity of care.
- Support the performance of service assessments/screenings and document members' care needs.
- Maintain and document member records in compliance with state and regulatory requirements, ensuring accurate distribution to providers.
- Adhere to standards of practice and policies in line with contractual requirements and regulatory guidelines.
- Identify member needs and make referrals to Care Managers, community-based organizations, and Disease Managers.
- Provide education on available benefits and resources.
- Perform other duties as assigned.
- Comply with all organizational policies and standards.

**Education/Experience:**
- High School diploma or GED required.
- 1-2 years of related experience preferred.
- Experience in a call center environment is a must.
- Knowledge of medical terminology is essential.
