Shift: Monday - Friday 9:00 a.m. - 6:00 p.m. EST
Pay Range: $25/hr on W2
Job Description: Care Coordination
**Position Purpose:**
The Care Coordination Support Specialist plays a key role in supporting care management activities and ensuring the delivery of services to members by healthcare providers and partners. This position helps ensure continuity of care and member satisfaction through proactive outreach and coordination. The Care Coordination Support Specialist will engage with members through phone calls or home visits to discuss care plans, address concerns, and provide education and resources to help manage their care effectively.
**Key Responsibilities:**
- **Member Outreach & Engagement:**
- Conduct phone or home visit outreach to members to discuss their care plans or service plans, including next steps, available resources, and any questions or concerns.
- Provide ongoing education to members throughout the course of their care or services.
- **Care Coordination:**
- Coordinate care activities with healthcare providers, community partners, and caregivers to ensure services are delivered according to the care plan or service plan.
- Work closely with healthcare professionals and members to accommodate changes or updates to care plans based on progress or new needs.
- **Support for Member and Provider Inquiries:**
- Assist with inquiries, requests, or concerns related to the care plan or service plan from members, healthcare providers, and partners.
- Ensure smooth communication among care managers, practitioners, and others involved in the care process.
- **Assessment & Documentation:**
- Support the performance of service assessments and screenings to document the member's care needs.
- Maintain accurate, up-to-date member records in compliance with state and regulatory requirements and distribute necessary documentation to providers.
- **Compliance & Standards Adherence:**
- Follow all organizational standards, policies, and procedures, ensuring compliance with contractual requirements and regulatory guidelines.
- Ensure confidentiality and privacy of all member data in accordance with relevant laws and regulations.
- **Referrals & Resources:**
- Identify member needs and make appropriate referrals to Care Managers, community-based organizations, or Disease Management programs as necessary.
- Provide members with education on available benefits and community resources to support their care.
- **Additional Responsibilities:**
- Perform other duties as assigned by the supervisor to support the overall success of the care coordination process.
**Education and Experience:**
- High School Diploma or GED required.
- 1-2 years of related experience in healthcare or care coordination preferred.
**Skills and Qualifications:**
- Strong communication skills with the ability to engage effectively with diverse members and healthcare teams.
- Knowledge of care coordination, healthcare services, and community resources.
- Ability to document member interactions and maintain accurate records in compliance with regulations.
- Problem-solving abilities and capacity to handle sensitive or challenging situations with empathy and professionalism.
