Position Title: Care Coordinator I

Location: Remote FL

Anticipated Start Date: 1/6/2025

Shift Time: 9:00 am - 6:00 pm

Expected Duration: [03 months]

Job Title: Care Coordinator I

Position Overview: The Care Management Support Specialist plays a key role in supporting care management activities, working closely with healthcare providers, community partners, and members to ensure services are effectively delivered, continuity of care is maintained, and member satisfaction is achieved. This role involves outreach to members, coordination of care, documentation of care plans, and communication with various stakeholders to ensure optimal care delivery.

Key Responsibilities:

Member Outreach & Engagement: Perform telephonic outreach or home visits to engage members in discussions about their care/service plan, including next steps, resources, questions, concerns, and ongoing education. Care Coordination: Coordinate care activities based on the member's care/service plan, working with healthcare providers, community partners, and members/caregivers to ensure any changes or progress are addressed. Member & Provider Support: Serve as a support resource for member or provider inquiries, requests, or concerns related to care/service plans. Team Communication: Collaborate with care managers, healthcare practitioners, and other team members to ensure continuity of care and facilitate timely and effective member services. Assessments & Documentation: Support in performing service assessments/screenings for members and maintain accurate documentation of care needs in accordance with state and regulatory requirements. Referral Support: Identify member needs and make appropriate referrals to Care Managers, community-based organizations, and Disease Managers as needed. Education & Resource Guidance: Provide education to members on available benefits and resources. Policy Compliance: Ensure adherence to standards of practice, policies, and contractual/regulatory requirements. Additional Duties: Perform other related duties as assigned.

Education & Experience Requirements:

High School diploma or GED required. 1-2 years of related experience in healthcare, member services, or a related field.

Skills & Qualifications:

Strong communication and interpersonal skills to effectively interact with members, healthcare providers, and team members. Ability to work independently and in a team-oriented environment. Familiarity with care plans, service coordination, and healthcare documentation standards. Knowledge of state and regulatory guidelines in healthcare. Ability to identify and address member needs and facilitate necessary referrals. Compassionate and empathetic approach to working with members.
