implements discharge planning within an interdisciplinary healthcare team addresses complex clinical care needs by identifying and removing barriers that prevent optimum access to needed post acute careWorks collaboratively with the providers to identify discharge barriers and delays in order to optimize most efficient use of acute care hospital days and prevent prolonged length of stayAbility to address all aspects of patientrsquos psychosocial needs including leading family meetings providing bereavement support and grief counseling advocacy with community agencies and within the acute care interdisciplinary team and addressing of substance abuseWorks collaboratively with all teams in the hospital to optimize patientrsquos quality of life by addressing patientrsquos needs reporting for Elderly and Mental retardationDemonstrate comprehension of medical terminology natural history of illness and general disease processes identification of and reliance on educational resources to continuously improve clinical practice as a medical social workerExcellent communication negotiation and conflict resolution skills requiredKnowledge of computer applications
