As the Care Coordinator, Social Worker (CC-MSW) you will be responsible for exemplary cross continuum care coordination, inclusive of psychosocial assessments and interventions. Position: Care Coordinator MSW Emergency Room primary with cross coverage to in patient units as needed

Shift: Days, 36 hours per week,7:00am- 7:30pm. 

Holiday Requirements: Rotational based on department needs.

Weekend requirements: Approximately every 3rd weekend. Weekend hours are shared among team members. 


Experience:
1. At least 5 years of recent clinical experience in a physician's office, clinic, hospital, or emergency services (EMS) setting, or other equivalent experience. Acute hospital or rehab or emergency room experience as a MSW is preferred. 
2. Leadership experience a plus.

Education:
Master's Degree in Social Work from CSWE (Council on Social Work Education) accredited program.

Licensures/Certifications:
Licensed Social Worker strongly preferred.
