Position Summary
The Care Coordinator (CC) is a non-clinical team member who facilitates patient continuity of care with the healthcare team. Under the clinical direction and oversight of the VP of VBC Operations, Primary Care Physicians and Registered Nurse, the Care Coordinator navigates patients at risk by collaborating with the patient and interdisciplinary healthcare team to serve needs and expedite appropriate, cost-effective care.
The PCC will support the clinical and operations teams in the care of patients. They will engage patients and take an active role in improving the health of patients we serve. They will partner with the care team in providing the patient with the tools necessary to make healthy lifestyle choices, adopt life-long healthy behaviors, and close gaps in care.
Duties and Responsibilities
The duties include, but are not limited to:
Assist clinicians on various tasks including follow up care, patient care notification, care gap closure and patient schedulingResponsible for engaging with patients and the care team to ensure patients are scheduled and complete annual wellness visits and follow up appointments as needed, per provider instructionsSupports the care plans for the patient and facilitates engagement between the providers and the patientsManages outgoing orders and incoming results and ensures accurate documentation and storage within the electronic medical recordInitiates all appointment reminder calls in a professional and courteous manner, projecting appropriate language and tone of voiceDemonstrates effective oral, listening, and written communication skills that facilitate positive customer relationships and staff interaction in the delivery of carePrioritizes duties and responsibilities and completes them accurately and in a timely fashion.Demonstrates responsible judgment by updating knowledge, referring problems through appropriate channels, and observing the policies and procedures.Upholds compliance with legal requirements, HIPAA, OSHA, and company policies.Assisting with other office duties including answering phones, faxing, filing, etc.Support individualized patient care plansAddress patient risk factors and/or obstacles to careFacilitate engagement between PCP and patientDemonstrated ability to influence others while motivating them to changeExercise sound judgment, seek advice and sign off from RN or LPN, as appropriatePerforms miscellaneous job related duties as assignedQualifications or Education, Training and Experience
Two (2)+ years of work experience in one or more of the following settings: Acute Care, Ambulatory, Specialty, Physician Office, Care Management or Home Health CareHigh school degree requiredCMA or MA preferredKnowledge of athenaOne preferred Must have valid driver’s license and ability to travel to doctors’ offices throughout NJ
