Care Coordinator SW\/LPN\/RN\n

Providing direct patient care coordination for diverse populations, in alignment with CHP network strategic goals and community partnerships. The Care Coordinator collaborates with providers (inpatient and outpatient), hospitals, home care agencies and additional community-based organizations to assure collaborative and consistent patient care delivery. Applicant should have strong personal, analytical, problem solving, critical thinking, organization and decision making skills. The Care Coordinator must possess the ability to function independently and have the flexibility and personal integrity to work in partnership with CHP network stakeholders and partners as assigned. Strong attention to detail with the ability to prioritize, coordinate, and simultaneously maintain multiple projects with high level of quality and productivity is a must.\n

Roles and Responsibilities:\n\nPromote the mission, vision, and values of Cayuga Health Partners.\nActively participates in CHP trainings and activities as assigned.\nAppropriately recognizes and responds to opportunities for improvement.\nPositively and proactively interacts with team members and network partners.\nCollects data as indicated to meet evaluation needs of the program.\nPrioritizes care programs as assigned and routinely updates practice integration plans. Appropriately documents all patient encounters as indicated.\nMaintains patient confidentiality in the provision of quality care.\nServes as a representative of CHP, CHS, and practices as assigned.\nParticipates in special projects in alignment with system and network goals.\nPractices always within scope - activities may be carried out within the role provided you have the knowledge, skills, and experience to do so lawfully, safely and effectively.\nActively participates in organizational and community committees.\n\n

Required Skills and Experience:\n

Education - Minimum Bachelors in SW, LPN Certificate, or Associate's degree (or higher) in the field of nursing.\n

Experience - Minimum 2 to 3 years' experience in an acute care hospital setting or alternate health care case management experience. Knowledge of community-based organizations and strong communication, organizational, critical thinking and problem solving skills required.\n

Licensure - Current unrestricted NY state RN licensure where indicated.\n

Physical Requirements:\n\nAbility to stand, sit or ambulate for long periods. Ability to mobilize freely around units, transport self to site locations, and perform required data entry into EMR and alternate data management systems. Ability to transport self to customer base.\n\n

Pay Range Disclosure:\n\n$32 to $45 per hour\n\n
