Under the direction of the Case Management leadership team, will work collaboratively with the Social Workers (SW), RN Care Managers (RN-CM), Utilization Review RN's (UR-RN) and Payer Denial Specialists by performing multiple administrative supportive duties that facilitate a seamless transition of patients from the inpatient hospital setting, using current knowledge of hospital processes and community services. This position requires frequent contact with patients and families including but not limited to hospital staff, community resource agencies and payer representatives. Will work collaboratively with the inpatient and outpatient case management teams to provide evaluation and assessment of patient needs. Duties involve both direct (in person) and indirect (via phone) patient care activities. This position requires the individual to take initiative, be proficient, and have strong decision-making skills, organizational skills, communication skills and a commitment to quality and excellence in service.

Required qualifications:
Bachelor's Degree. 2 years Experience in customer service, patient education, and/or patient engagement.Preferred qualifications:
1 year Experience in a healthcare setting. 1 year Experience in managed care of case management.
