The Care Management Assistant works under direction to provide assistance with the utilization review process such as intake, transmission and tracking of clinical reviews to payors, assistance with coordination of post hospital care and service arrangements. Also provides assistance to ensure that all documentation, communication and information required to facilitate patient discharge is complete.
Your every day
Clerical Support for Discharging Patients: Participates in discharge planning activities as needed to ensure appropriate referrals to post-acute providers and timely patient discharges. Assists in the research for available preferred providers for post-acute care to meet the patient’s needs for discharge. Contacts post-acute care providers and transmits information needed for the referral process. Provides follow up to ensure identification and communication of available resources. Collects, copies, and transmits pertinent patient demographic and clinical information required to complete arrangements for post-acute care and/or placement, as directed. Transmits transition of care documents to primary care physician and post-acute providers.Clerical Support for the Utilization Management Process: Facilitates timely transmission of reviews to the payers in coordination with the case management team. Communicates any pertinent information received from payers to relevant stakeholders. Supports and assists with concurrent insurance denials and appeals processes. Provides denial and non-certified day information to the appropriate stakeholders to reduce potential denials. Collaborates with other departments to ensure accuracy of patient and payer information.Department Assistance: Assists the department leadership in data management. Updates payer contact information in software system to ensure accuracy in review submission. Maintains accurate payer contact information to ensure accurate and timely communication. Combination of appropriate education and work experience is required:High School Diploma/GED and 2 years of office/clerical experience in a healthcare setting.4 years of experience is required in lieu of education.WORK SHIFT:
