Job Description:This position supports the RN Care Coordinator by managing patient care plans, utilizing excellent organizational and communication skills, and showing a personal commitment to providing compassionate care.

Key Responsibilities:

Manages patient care plans, including developing and updating plans as neededProvides communication with patients, care teams, and other stakeholders to ensure that all relevant information is available to provide the best possible quality of careAssists with scheduling appointments, coordinating care, providing follow-up services to patients, enters orders and authorizations, submits data to health plans, and any other duties as assignedTracks and maintains patient records, including medical history, medications, and insurance information. Ensure that all patient care plans follow applicable regulations and guidelines. Follow up with patients to ensure their care plans are being followed and that they are receiving the best possible careProvides administrative support including data entry, documentation in the Electronic Medical Record, and other duties as requiredCollaborates with other departments to ensure the efficient delivery of care

Licensure/Certification Qualifications:

Certification/Licensure Requirements:

Current licensure as a Licensed Practical Nurse in the following states: Minnesota, Wisconsin, North Dakota

OR

Current registration or certification as a Medical Assistant OR Verifiable completion of a Medical Assistant Program and must become certified within one year of date of hire. (Must pass certification within two attempts).
