Care Management Educator

Summary

Role Overview:

Directs, plans and evaluates educational or training programs by helping to ensure the competence of Care Management Nursing, Social Work and Administrative Staff and to foster the delivery of evidence-based practice.

 

DUTIES & ESSENTIAL JOB FUNCTIONS:  NOTE: Following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Not all duties listed are necessarily performed by each individual in the classification.

Audits the patient record to assess the need for education and medical staff intervention. Assures documentation is evident that the patient has received optimal care according to the evidence-based standards, intervening with CM leadership and staff when necessary. Collaborates and communicates regularly with Care Management Systems analysts, IT, contracted Health Plans, Medical/Provider Groups, industry wide organizations (i.e. Department of Managed Health Care, Alameda County Health department, California Children’s Services), internal departments and any other providers when appropriate. Collaborates with Human Resources on any training programs available to staff. Develops and applies appropriate measurement tools in quality or process improvement activities. Evaluates new information and determines what is needed to comply with mandates. Summarizes findings from data and makes recommendations for corrective action plans, when appropriate, to the appropriate department manager/director. Facilitates all Care Management trainings especially when it impacts the work flow of staff or will be implemented within the Care Management Division. Facilitates the initiation and adoption of changes in multidisciplinary clinical practice by providing a consulting role in the change process. Manages all Care Management comprehensive staff education and training plan, including annual competencies, application implementation, and other competencies. Perform all other duties as assigned. Plans, prepares and maintains materials for use in educational or training programs for accuracy and relevance to the target audience. Provides reports to department managers/directors to document staff compliance and progress of required competencies. Responsible for division education plans that support annual Organization Goals and strategic plan. Reviews annual Care Management staff (clinical and administrative) educational needs assessment for the upcoming year, analyzes and prioritizes information, and builds a document/plan that is shared with leadership each year. Teaches content as needed and/or assigned within clinical expertise in formal and informal settings with a focus upon improvement of skills and retention of knowledge using appropriate strategies, technologies and adult learning principles.

 

MINIMUM QUALIFICATIONS:

Preferred Education: Master’s in Nursing.

Preferred Licenses/Certifications: Certification in Case Management, CCMC or ACM, and/or certified as an Interqual Instructor.

Required Education: Bachelor’s of Nursing.

Required Experience: Five years Case Management experience in an acute setting.

Required Licenses/Certifications: Active licensure as a Registered Nurse in the State of California, Active BLS Basic Life Support Certification issued by the American Heart Association. Other advanced life support certifications may be required per unit/department specialty according to patient care policies. CPI -Crisis Prevention Intervention Training (required for all positions at John George Psychiatric Pavilion and certain positions in the Emergency Department).
