 The Care Operations Support Assistant contributes to administration of care management. Provides non-clinical support to the assessment and evaluation of patient’s needs and requirements to achieve and/or maintain optimal wellness state by guiding patients/families toward and facilitate interaction with resources appropriate for the care and wellbeing of patients. The Care Management Support Assistant 3 performs advanced administrative/operational/customer support duties that require independent initiative and judgment. May apply intermediate mathematical skills.

Schedule: Full-time, M-F, 8a-5p

The Care Operations Support Assistant - Decisions are typically focus on methods, tactics and processes for completing administrative tasks/projects. Regularly exercises discretion and judgment in prioritizing requests and interpreting and adapting procedures, processes and techniques, and works under limited guidance due to previous experience/breadth and depth of knowledge of administrative processes and organizational knowledge. Generate and maintain KPIs and daily reports for care management activities. Schedule and confirm patient appointments efficiently and accurately. Conduct urgent appointment setting and follow up calls to ensure patients receive timely care. Work cross- functionally with external entities for urgent appointment coordination. Accurately maintain electronic medical records (EMR) by entering relevant data and documents. Manage and prioritize patient waitlists to ensure fair and efficient care allocation. Facilitate the pairing of Medical Assistants with Clinicians to ensure smooth patient care delivery. Attend and actively participate in clinical team meetings to discuss patient care strategies and quality improvement initiatives.

Required Qualifications

This position is Remote, GA and requires the candidate to reside in the state of Goergia due to business needs.

Minimum of 1 - 3 years of experience in Provider Relations, Operations, Customer Service, Logistics, and/or Project Coordination.

1- 2 years of demonstrated leadership experience.

Minimum of 1 - 3 years of technical experience working within multiple operating systems and platforms at any given time. Knowledge of Microsoft Office Word, Excel, and Outlook.

Administration support experience in a healthcare industry with familiarity with care and well-being resources.

Excellent verbal and written communication skills including telephone and customer service etiquette.

Ability to work cross-functionally and communicate effectively with team members, patients, clinicians, customers, and leaders of all levels.

Excellent interpersonal skills, attention to detail, time management, and problem-solving skills.

Experience working with patient electronic medical records (EMR). 

Must be passionate about contributing to an organization focused on continuously improving consumer experiences.

Preferred Qualifications

It is preferred that the candidate reside in the Atlanta, GA or the Atlanta surrounding area.

Associate or bachelor’s degree in business or a related field.

2 or more years of demonstrated leadership experience.

Experience with Microsoft Access and Microsoft Teams.

Knowledge and/or experience with value-based care and primary care.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
