Job Title: Care Management Support Coordinator IILocation: Remote (MST preferred, EST acceptable with strict shift adherence)Duration :6+ months Pay Rate: $24/hourTarget Start Date: 04/07/2025
Position Purpose:The Care Management Support Coordinator II provides administrative support for care management activities, including member outreach, inbound call handling, and scheduling services. This role serves as a key point of contact for members, providers, and internal staff to address inquiries and ensure regulatory compliance in documentation and service coordination.
Key Responsibilities: Conduct outreach calls to members to discuss care plans, available resources, and next steps. Address member and provider inquiries regarding care coordination, scheduling, and educational resources. Assist members in connecting with health plan and community resources to ensure high-quality service. Maintain non-clinical member records and documentation in compliance with regulatory standards. Support member onboarding by sending welcome letters, educational materials, and other relevant correspondence. Work within multiple systems while handling inbound call queues efficiently. Ensure timely and accurate documentation of all interactions in Virtual Health. Provide front-line support to explain care plan procedures, protocols, and next steps. Monitor Social Determinants of Health (SDOH) needs and make necessary referrals. Comply with all organizational policies and regulatory requirements. Perform other administrative duties as assigned.
Qualifications & Requirements:Education:High School Diploma or GED required.Experience: Minimum 2 years of inbound call center experience (Mandatory). Customer service experience, with strong listening and documentation skills. Ability to multitask and navigate multiple systems simultaneously. Technical Skills:Basic understanding of Microsoft Office applications.
Work Schedule & Expectations: Work Location: Remote (MST preferred; EST candidates must adhere to the designated shift). Performance Metrics: Handle at least 80 inbound calls per month. Maintain an average call answer speed of under 30 seconds. Meet monthly performance and customer service benchmarks.
