Care Manager Rochester, NY (http://maps.google.com/maps?q=1900+South+Clinton+Ave+Rochester+NY+USA+14618) Description JOB TITLE: Care Manager DEPARTMENT:Caregiver PROGRAM:Finger Lakes Caregiver Institute (FLCI) SUPERVISOR:FLCI Program Manager, Care Management STATUS:Full Time or Part-time,Non-exempt Hourly Rate: $20.00 to $22.00 SUMMARY The Finger Lakes Caregiver Institute (FLCI) is a specialized program at Lifespan serving caregivers of older adults. FLCI Care Managers focus on serving caregivers of individuals with memory loss or forms of dementia. FLCI Care Managers help caregivers by providing unbiased information about care options and linkage to services for their care partner as well as support, education and programs for themselves as a caregiver. Care Managers offer a holistic assessment of the caregiver’s and the person living with memory loss’ needs by identifying health, financial, emotional, functional and environmental concerns. Care Managers offer care options, solutions and guidance navigating challenging systems. Care Managers may facilitate support groups and assist with caregiver education events. Services are provided in the 9 County region of Monroe, Chemung, Livingston, Ontario, Seneca, Steuben, Schuyler, Wayne and Yates counties. DUTIES AND RESPONSIBILITIES 1.0 Provide care management to caregivers and their care partners. 1.1 Accepts referrals from various sources including, physician practices, NY Connects, Area Agencies on Aging, self-referrals, etc. 1.2 Completes comprehensive assessments to determine caregiver goals. 1.3 Creates and implements care plans that meet caregiver needs and preferences. 1.4 Reassess goals and modifies care plans as necessary. 1.5 May provide information and referral, advocacy, benefit and entitlement counseling. 1.6 May complete or assist with Medicaid or other benefit applications. 1.7 Connects caregivers and clients to the Alzheimer’s Association. 1.9 Completes all documentation within required timeframes. 2.0 Arranges mental health interventions for caregivers and their care partners when necessary. 2.1 Conducts mental health screenings when appropriate. 2.2 Provides supportive counseling to caregivers and care partners. 2.3 Works together with the physician office care team to provide linkage to appropriate ongoing mental health community services. 3.0 Participates as a member of the inter-agency team. 3.1 Participates in all FLCI staff meetings and training. 3.2 Participates in all Lifespan staff meetings and trainings. 3.3 Participates in Quality Assurance activities. 3.4 Accepts and seeks guidance and clinical supervision. 3.5 Collaborates with Lifespan team members and professional partners. 4.0 Timely Completion of all required documentation and data entry into appropriate database. 5.0 Maintains up to date professional knowledge of the geriatric field, with special focus on Alzheimer’s Disease and Dementia, care options, community services, entitlement benefits, clinical skills and competencies through reading, workshops and conferences. 6.0 Is aware of and actively supports the Agency’s initiative of being collaborative and community focused, innovative. Understands, demonstrates and values the ideals of diversity, equity and inclusion in all aspects of their position. 7.0 Other duties as assigned by supervisor. Requirements QUALIFICATIONS EDUCATION:Bachelor of Social Work or equivalent with 2 years of experience. EXPERIENCE:Two years in social work, with at least one year of experience working with caregivers and persons with dementia, geriatrics or comparable human services experience is preferred. Experienced with competing Medicaid applications a plus.Bilingual Spanish a plus. PHYSICAL AND MENTAL REQUIREMENTS · Must be able to meet with clients in their homes or the greater community. · A portion of the work is performed at a desk or at a personal computer station. · Verbal, written and telephone communication skills are required. · Strong written and computer skills needed. · Interface with all levels of personnel. · Ability to maintain composure under strict deadlines and difficult situations. · Ability to work in multi-cultural situations. · Access to reliable transportation. · Valid NYS Driver’s license Core Competencies + Empathy: there are so many points of stress and anxiety for caregivers; caring for a loved one, experiencing end of life transition, coping with medical challenges and finances, and managing a family and often career. Being an empathetic and compassionate CM is vital to success. + Mental Health: Perhaps the biggest challenge facing caregivers is its impact on their mental health and how that affects their physical health.
