SCHEDULE: Monday-Friday, 8:00 AM- 4:30 PM
The enhanced care management (ECM) program facilitates client integration, improves quality of care, and promotes a safe home environment. This state-funded pilot program supports clients through increased access to a case manager, access to education and resources, and frequency of home visits. Medicaid Fee-for-Service, Managed Medicaid, and dual Medicare-Medicaid eligible adults who are medically ready to transition from acute care but are unable to due to transition barriers are eligible for enrollment in this service.
This role serves as a hospital-based team member designated to coordinate patient-specific social, behavioral, and medical services. They will also serve as a liaison and patient advocate with the Department of Social and Health Services (Developmental Disabilities Administration case managers and Home and Community Services case managers), other hospital staff, managed care organizations, transition of care coaches, outpatient providers, and tribes and/or tribal liaisons.

Required Qualifications:
Master's Degree Social Work Upon hire: Washington Clinical Independent Social Worker Associate License 1 year Experience in discharge planning, care coordination, or community resource navigation, in a clinical or community setting.Why Join Providence?
