Job Details CARE MANAGER RESPONSIBILITIES: Assesses, plans, implements, monitors and evaluates options and services to effect and appropriate individualized plan of care for patients across the acute care continuum. Performs required activities for utilization management, PRI assessment and completion, transition planning, and identification of actual or potential quality/risk indicators, in compliance with regulations and organizational goals. Performs as an integral part of the interdisciplinary team to effectively ensure optimal patient outcomes and length of stay efficiency. Considers payer requirements and responds accordingly to avoid adverse financial consequence to patient and hospital. Works proactively with ED staff, ED charge nurse, and ED physicians to identify possible admissions. Determines DRG-based initial LOS values for ED admissions (all status/types). Partners with ED nurse, ED charge nurse, and ED physician to communicate with patients about their admission and plan of care. QUALIFICATIONS: Graduate of an accredited school of nursing required. Bachelor’s Degree in Nursing (BSN) preferred. Current New York State RN License required. BLS required. Minimum of 3-5 years’ relevant clinical medical/surgical or specialty experience required. 2-3 years’ Utilization Management, Performance Improvement, Transition Planning and/or Care Management experience in an acute hospital or related managed care setting required. Salary Range USD $56.10 - USD $62.10 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
