\nJob Summary
\nAs Care Manager RN 1, you oversee patient care, ensuring quality and integrating services. Using standards, you will manage benefits and justify service necessity. This role combines healthcare practitioner, case manager, and educator duties to achieve top clinical outcomes. By controlling care and resources effectively, unnecessary service utilization can be reduced.

\nSetting: Case Management - 65+ older population
\nSchedule: FT Monday-Fri - onsite in the clinic
\nWhat You'll Do
\n\nAssess the patient's care plan. Document, execute, and update collaboratively with the healthcare team to deliver evidence-based care.\nConstruct patient care management strategies, treatments, and self-management priorities.\nEvaluate the healthcare, educational, and psychosocial requirements of the patient and their family.\nHelp patients manage health autonomously through education, in-person visits, and remote engagement. Encourage patients to follow care plans.\nExplore, assess, recommend resources for medical and non-medical needs of patients and families.\nUnite with all programs for service coordination.\nIdentify and address problems in the care coordination, exploring potential solutions to ascertain the most suitable care setting.\nServe as a liaison to hospitals, general care providers, post-recovery partners, and specialists.\n\nWhat Will Make You Successful In The Role
\n\nProficiency in conducting patient assessments and managing their care is critical for this role.\nExceptional verbal and written communication skills are paramount.\nThey should possess the ability to diligently monitor, evaluate and document patient progress with respect to personalised care plans.\nThey should have the capability to facilitate a patient's access to appropriate community resources.\nWe must work well with internal medical team and external partners and organizations.\nTogether, successful partnership is crucial.\nThey must have a flexible capability to evaluate, adjust and respond positively to a shifting and crisis-filled environment.\nIt is crucial to respect all aspects of patient confidentiality, in compliance with all relevant policies and regulations.\n\nBelonging Statement
