Job Summary

\n

As a Care Manager RN 2, play a vital role in patient care. You assess and coordinate care using guidelines to manage services effectively. You are a care provider, case manager, mentor aiming for quality outcomes by managing resources efficiently.

Location: McKinney, TX - 5252 W University Dr, McKinney, 75071

Setting: Case Management

\n

Schedule: FT Mon-Fri

\n

What You'll Do

\n\nYou will manage care plans, working with our health care team and using medical best practices.\nYou will establish patient care management plans. This includes crafting interventions, establishing treatment goals, and setting self-management goals.\nYour role is to assess the needs of the patient and family, including healthcare, education, and psychosocial needs.\nYou will help patients manage health conditions, educate through visits, calls, and guide adherence to care plans.\nYou will research and recommend resources for various patient needs, medical and non-medical. Your role includes coordinating services between programs smoothly.\nYou will earnestly partake in identifying and problem-solving care coordination inefficiencies, thereby facilitating optimal care.\nYou will mediate between hospitals, primary caregivers, specialists, and post-acute partners in your role.\nYou'll guide staff, improve processes, and address questions as a skilled in care management workflows.\nYour constant effort towards staff retention, which includes responding promptly to their concerns, will be highly valuable.\nYou will oversee new staff orientation, support preceptors and novices, evaluate performance, and provide feedback to managers. Participating in staff interviews is important.\n\n

What Will Make You Successful In The Role

\n\nA specialty certification would be advantageous.\nBeing knowledgeable about care management, resource allocation, and utilization management will be key.\nYou should possess skills in assessing and managing patient care.\nEffective verbal and written communication skills are essential.\nYou should have strategic, critical thinking capabilities for problem-solving and addressing concerns.\nYou will need the ability to monitor and record patient progress against a care plan.\nYou should be able to assist patients in accessing community resources.\nYou must cooperate with everyone in medical field, inside and outside organization, for success.\nYou should be able to assess, adapt to, and respond calmly to a fluctuating and crisis-prone environment.\nYou'll need to maintain confidentiality regarding all aspects of patient info, conforming to all pertinent policies and regulations.\n\n

Qualifications

\n\nAssociate Degree\nA minimum of (3) three years of relevant work experience\nMust possess a current and valid Registered Nurse license\n
