Per Diem/Day Shift 

RN Care Managers, in the acute care setting, provide a variety of services to appropriately help progress patients through the continuum of care.  Key services include screening for post-acute needs; transition planning; care coordination; education; community care referrals; care planning in accordance with regulatory requirements; collaboration with the continuum care team; ongoing monitoring of plan effectiveness and reassessment of the need for Care Management (CM) services.

Required qualifications:

Nursing degree/diploma required upon hire. Bachelor's degree in Nursing or higher within 3 years of hire. Upon hire: Oregon Registered Nurse License  2 years of Acute care hospital or community based experience. 1 year of experience with electronic medical records or documentation systems. 1 year of Home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience

Preferred qualifications:

Bachelor's degree in nursing upon hire.     Upon hire: CCM certification
