The RN Care Manager is responsible for carrying out effective case management functions for identified patients by planning, coordinating, negotiating, procuring and managing the care and needs of complex patients and their families throughout the continuum of care.

Required qualifications:

California Registered Nurse License upon hire. 2 years of Care Manager RN-Combined current clinical experience in public health, community health, or in an acute care hospital setting OR recent completion of a post-graduate level curriculum focusing on case management 2 years of Care Manger RN Mental Health-Combined current clinical experience in Mental Health, Community Health, or in an Acute Psychiatric Hospital setting.

Preferred qualifications:

Bachelor’s Degree in Nursing Community Based Care Management including working with individuals who are homeless or at risk for homelessness, with mental health conditions or substance abuse. Psychiatric/Mental Health Nursing Experience.
