Our expansion, spanning 165,000 square feet, will introduce a state-of-the-art patient tower to our Round Rock hospital, tailored to accommodate the evolving needs of our rapidly growing population. Among its highlights, the tower will house an innovative 11-bed Neonatal Intensive Care Unit, alongside expanded capacities for crucial departments such as med/surg and labor and delivery.

Anticipate a host of enhancements, including:

\n\n Additional operating rooms, cath labs, and procedural areas.\nExpansions and upgrades to communal spaces.\nImplementation of advanced equipment across various departments.\nShell space earmarked for future growth, including the expansion of our Emergency Department and operating suites.\n\n

\n

Job Summary

\n

You'll oversee patient care, using guidelines to dictate benefits and resolve service necessity issues. Various roles will be adopted to encourage optimal outcomes. Manage care efficiently to reduce unnecessary usage.

\n

What You'll Do

\n\nProducing, implementing, documenting, and updating care plans in collaboration with the entire healthcare team\nFormulating patient care management plans and setting treatment and self-management goals.\nEvaluating the healthcare, educational, and psychosocial requirements of the patient and their family.\nHelping patients stay healthy by educating, visiting, and communicating via phone; also, supporting adherence to care plans.\nStudying, establishing, and recommending resources to fulfill medical and non-medical needs of patients and families.\nCollaborating with all programs to ensure proper coordination of services.\nProactively identifying issues and suggesting solutions for care coordination to ensure it's in the best setting.\nCommunicating and linking with hospital, long-term care, and home health representatives.\n\n

What Will Make You Successful In The Role

\n\nCompletion of an associate degree in nursing.\nCompletion of a specialty certification is highly recommended.\nFamiliarity with care coordination, managing resources and handling patient needs effectively.\nProficiency in assessing patient health and creating effective management plans accordingly.\nOutstanding verbal and written communication skills.\nAbility to diligently monitor, evaluate and document patient progress in line with a care plan.\nCapability to aid patient access to community resources.\nWillingness to collaborate with both in-house and external medical professionals, partner agencies and organizations.\nAbility to handle rapidly changing situations and potential crises with understanding and composure.\nCommitment to ensuring confidentiality of patient details in line with all relevant policies and regulations.\n\n
