Job Title: Care Manager (RN)Job Description
The Care Manager (RN) develops, assesses, and facilitates complex care management activities for members with primarily physical needs to provide high quality, cost-effective healthcare outcomes. This includes creating personalized care plans and educating members and their families.
Responsibilities
+ Evaluate the needs of members, identify barriers to accessing appropriate care, and address social determinants of health.
+ Develop ongoing care plans and collaborate with providers to identify specialists and community resources to meet members' unmet needs.
+ Identify problems and barriers to care, and provide appropriate care management interventions.
+ Coordinate between members, families, caregivers, and care provider teams to ensure members receive appropriate person-centered care.
+ Provide ongoing follow-up and monitoring of member status, changes in condition, and progress towards care plan goals.
+ Revise or update care plans as necessary to meet members' goals and unmet needs.
+ Provide resource support to members and care managers for local services based on assessments and plans.
+ Facilitate care management and collaborate with providers or specialists to ensure timely access to needed care or services.
+ Perform telephonic, digital, home, and other site outreach to assess member needs and collaborate with resources.
+ Collect, document, and maintain all member information and care management activities to ensure compliance with regulations.
+ Provide and/or facilitate education to members and their families on disease processes, resolving care gaps, healthcare provider instructions, care options, referrals, and benefits.
+ Provide feedback to leadership on opportunities to improve and enhance care and quality delivery in a cost-effective manner.
+ Perform other duties as assigned to meet business needs.
+ Comply with all policies and standards.
Essential Skills
+ Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing.
+ 2-4 years of related experience.
+ RN - Registered Nurse - State Licensure and/or Compact State Licensure.
Additional Skills & Qualifications
+ Experience in care management and working with members on complex care needs.
+ Strong communication and collaboration skills.
+ Ability to perform telephonic, digital, home, and other site outreach.
Work Environment
The work environment includes telephonic, digital, home, and other site outreach to assess member needs. The role requires compliance with state, federal, and third-party payer regulations and involves working with various technologies and documentation systems. The dress code and specific facility conditions will be provided by the employer.
Pay and Benefits
The pay range for this position is $40.00 - $45.00/hr.
