We have an opening for a Full-Time Care Manager RN Coach to work at our employer health center located in Lisle, IL!

Schedule: Mon, Tue, Wed & Fri 8 am - 5 pm; Thu 10 am - 7 pm

PURPOSE OF THE POSITION:  RN position that leverages powerful data insights to proactively identify and engage members who need support the most.  Nurses will design individualized care plans, offer self-management education and coordination of resources that assists members to achieve improved health. Collaborates closely with clinical teams to provide guidance on engagement strategies, focused on improving health status of members. Manages a panel of high-risk members with chronic conditions to provide frequent touchpoints and ensure care gap closure as defined by national standards. Facilitates Care Team meetings to ensure an interdisciplinary approach to care for member panel.  Delivers one on one and/or group coaching sessions in the areas of weight, nutrition, physical activity, stress, tobacco cessation, work/life balance and general health. Works with members one-on-one or in small groups to assist in meeting health and wellness goals related to chronic condition self-management, weight, nutrition, exercise, stress, tobacco cessation, work/life balance and general health. Assists the clinical team with coordinating care for high risk members.

ESSENTIAL JOB FUNCTIONS 

Develops personalized treatment plans using provider care plans and member preferences using a coach approach and the pillars of Lifestyle MedicineSupports a proactive, multidisciplinary team approach directed toward prevention, education, and health promotionProvides condition specific self-care education to members enrolled in the various condition management programsParticipates in outreach/follow-up phone calls and follow-up member visitsDocuments touchpoints and progress into the electronic medical recordCollaborates with the full scope of clinical team members to arrange for further medical treatment when neededStrives to work with member to improve health outcomes and better management of any chronic conditionDelivers one on one and/or group coaching sessions in the areas of weight, nutrition, exercise, stress, tobacco cessation, work/life balance and general healthWorks closely with client’s vendors as appropriate to support member’s needs where appropriateIdentifies emergency situations and provides care within legal scope of practiceActs as facilitator of Care Team meetings.Coach effectively in-person and by phoneMay coordinate and conduct wellness programs in areas such as nutrition, stress management, weight management, smoking cessation, exercise and others

BASIC QUALIFICATIONS

Education: 

Currently licensed Registered Nurse in state of practiceCurrent Wellness Coach Certification from Wellcoaches® OR equivalent International Coach Federation-accredited program certification (must be obtained within 24 months of hire)Current hands on training in AHA or ARC Basic Life Support for health care providers required; Advanced Cardiac Life Support may also be required based on contract scope of service.Bachelor's degree (BSN) from four-year college or university (preferred)

Experience: 

At least three years’ experience as a Registered Nurse caring for members with any of the following conditions: Type 2 diabetes, asthma, heart disease, heart failure or hypertension.Experience working as a wellness coachExperience in Condition Management, Certificate in Case Management, Care Coordination, Certified Diabetes Educator (CDE) Certified Diabetes Care and Education Specialist (CDCES) a plus (preferred)Experience in health promotion or work site wellness and public speaking (preferred)Experience with Electronic Medical Records (preferred)
