The case manager performs the primary functions of assessment, planning, facilitation and advocacy, which are achieved through collaboration with the client and other health care professionals involved in the client's care.

Required Qualifications:

Bachelor's Degree Upon hire: California Registered Nurse License 2 years Acute hospital experience, including one year of supervisory experience.

Preferred Qualifications:

Master's Degree In health care related field.
